six (6) months from the date they were generated
Most Quest laboratory orders expire six months from the date of generation. However, dated pathology request forms do not have an expiration date. You can request a new order after six months, but you must pay a $15 reactivation fee to receive a new dated pathology request form. If your Quest laboratory order expires before you receive it, you can reactivate it by contacting customer service. You may also be required to fill out a new HIPAA form and log into your MyDLS account to reactivate the order.
In some cases, Quest may request additional information from you. These prompts are called Ask on Orders and appear below the selected Quest test. You must enter valid information to submit a lab order. Quest system will print a label and return a requisition form to your practice. Once you have confirmed the test and requisition form, you can send the lab order to your patients.
Electronic requisition form
When submitting a blood work order to Quest Diagnostics, you should print and attach a specimen label if you plan on billing the patient. After you’ve completed the requisition form, you can specify who will bill the patient for the service. The doctor, the patient, or their insurance company, if the patient has coverage. You can also set the default billing option, or customize the process to suit your needs.
If you’re unfamiliar with Elation’s Electronic Requisition Form (ERF), you can use it to place a blood test order. Then, it will be sent directly to Quest Diagnostics’ system. Moreover, a patient can ask the lab technician to find the order if they are unable to find it. This eliminates the need to print lab orders for patients, and lessens the risk of misplacing the paper order. Once you’ve completed the electronic requisition form, you’ll be prompted to select your Billing Type, so that Quest will know which blood test to perform and what to send.
Refund policy
The Quest Diagnostics company has been sued for allegedly double billing patients and sending duplicate claims for the same tests. The company is accused of ordering duplicate tests and up-coding tests in order to overbill Medicare and other payers. The company maintains its blamelessness, citing a technological glitch. The settlement amount is one hundredth of one percent of Medicare’s annual budget. Patients should check with their insurance companies to determine what they can and cannot receive reimbursement for blood tests ordered at the lab.
If you cannot get your blood test results, you can cancel your order by using QuestDirect. To cancel an order, click on the Cancel button twice in the order information section. The Cancel button will cancel your appointment and any unused portion of your order. It will take about three business days for the refund to reflect in your account. If you receive a refund for your blood test, you will have to wait seven days before submitting another request.
